Active Employees New to Vantage's Regional HMO Plan can go to their agency’s HR Department to complete a 2011 Annual Enrollment Form.
Retirees can go to their agency’s HR Department or to an OGB field office, or write a letter to OGB indicating their desire to change to the Vantage Medical Home Plan. The letter must include their name and social security number, and must be signed & dated by the OGB Plan Member on or before the end of the enrollment period.
